Health Requirement Exclusion Notification
This is to remind everyone that Friday, September 17 is West Central’s exclusion date for students who are not compliant with IDPH immunization and health record requirements such as physicals, dental and eye exams, and immunizations. Each year, we must certify to the Illinois State Board of Education that ALL of our students are in complete compliance with all requirements. By law, students who are not in full compliance with all IDPH immunization and health record requirements by the set exclusion date (September 17, 2021) must be excluded until such time that they are in full compliance.
If your child(ren) is not in compliance with these requirements, we have sent a letter. If you have documentation that your child(ren) have met the requirement that our records show as a deficit, please get that documentation to our District Nurse, Paige Torrance, by the end of the day on Thursday. If your child needs to be seen by their physician, please have your provider fax the school proof of your scheduled appointment to (309) 627-2453, and that will be sufficient until their appointment time. Those students not in full compliance by the end of the day Thursday, September 16 or who do not provide proof of a scheduled appointment, will be excluded from school on Friday, September 17, 2021, until such time as they are in full compliance.
