Eagle View Community Health System is hosting two Mobile Clinic Days this summer to help families easily complete their required school physicals, sports physicals, and dental exams. Check out the dates and locations below to find the clinic that works best for you.
📅 Mobile Clinic Schedule & Locations
To accommodate our families, the mobile clinic will be visiting two different locations on two separate dates:
Clinic Day 1: West Central Middle School

Time: 12:00 PM – 4:00 PM

Clinic Day 2: West Central High School

Date: August 3rd

Time: 11:00 AM – 1:00 PM

🩺 Services Provided

School Physicals: Required if your child is entering Pre-School, Kindergarten, 6th grade, or 9th grade.

Dental Exams: Required if your child is entering Pre-School, Kindergarten, 2nd grade, 6th grade, or 9th grade.

Sports Physicals: Available for $15.
Important Note on Insurance: If you have health or dental insurance, please remember to bring your insurance card with you to your appointment.
